Medication Therapy Management (MTM) is a service provided by a pharmacist to review all of a patient's medications, including prescription medications, over-thecounter medications, herbal products and dietary supplements. This review can help the patient understand how medications manage health conditions, answer questions concerning medications, check to see if there are any problems with medications such as overuse, underuse, duplication, side effects or interactions. If problems are found, the pharmacist can work with the health care provider to help solve these problems. Medication organizational help is also provided by creating lists of medications, compliance support and keeping track of medication histories and issues that can be shared with other health care providers.
